To ensure your first dental visit is smooth, download and fill out our new patient forms prior to your appointment to speed things up when you arrive at our dental clinic.

When you’re done, you can submit the new patient form to us online or in person.

To submit your new patient form online:  Click CHOOSE FILE and attach your completed patient form.  Click SUBMIT to send it to us. We will take care of the rest.
