At Perfect Smile Dental Group in Whittier, we are always looking for ways to help our patients get the dental treatment they need no matter whether they have insurance or not. We want you to make our dental clinic your dental home and for you to be a part of our dental family.

Perfect Smile In-Office memberships are provided exclusively to our patients without insurance and are not considered as pre-payment for future services or for access to discounted services. Membership discounts may not be combined with other offers or discounts. It is the patients’ responsibility to schedule and keep their appointments. No refunds will be provided for dues paid under any circumstances, including failure to schedule and failure to maintain appointments.
Frequently Asked Questions
How do I know if I need Adult Membership or Perio membership?
During your initial exam, the dentist will inform you if you have gum disease. If you were diagnosed with gum disease, you will need the Perio Membership. Deep Cleaning is not included in Perio Membership.
Can I pay the In-Office membership with credit card?
Payment for membership can be made with Credit card, debit card, or cash. Membership payment is paid in full for the whole year.
When can I start using my membership?
Immediately.
What happens if I miss an appointment?
It is the patients’ responsibility to reschedule the appointment. Please read our office policy regarding no-shows and last-minute cancellations. If you are not able to make it to your appointment, please give us a call 48 hours prior to reschedule to avoid last-minute cancellation fee.
Can I transfer my membership to another office?
Your In-office membership is only with our dental clinic and is non-transferrable. If you move, you can cancel your membership.
If I cancel, do I get a refund on my membership?
No refund will be provided for dues paid.
What type of documents are required to sign up for membership?
ID Card, Credit Card or Debit Card, Phone number, Address and Email Address.
